11. Tóm tắt các kết quả mới của luận án

- Mục đích nghiên cứu :

 Nghiên cứu hệ thống cơ sở lý luận về dịch vụ công tác xã hội trong bệnh viện và khảo sát những khó khăn và nhu cầu hỗ trợ của người bệnh và người nhà người bệnh trong quá trình khám chữa bệnh; đồng thời đánh giá thực trạng và phân tích các yếu tố ảnh hưởng đến việc triển khai ba nhóm dịch vụ công tác xã hội trong bệnh viện theo Thông tư 43/2015/TT-BYT. Trên cơ sở đó, luận án đề xuất các khuyến nghị nhằm hoàn thiện và nâng cao chất lượng dịch vụ công tác xã hội trong hệ thống bệnh viện trên địa bàn tỉnh Nghệ An.

- Đối tượng nghiên cứu: Dịch vụ hỗ trợ tư vấn người bệnh/người nhà người bệnh; dịch vụ hỗ trợ nhân viên y tế và dịch vụ truyền thông – giáo dục sức khỏe tại các bệnh viện ở Nghệ An, theo khung nhiệm vụ quy định tại Thông tư 43/2015/TT-BYT.

Thứ nhất, Kết quả nghiên cứu cho thấy các dịch vụ công tác xã hội trong bệnh viện tại Nghệ An đã được triển khai theo ba nhóm quy định tại Thông tư 43/2015/TT-BYT, bao gồm hỗ trợ – tư vấn cho người bệnh và người nhà, hỗ trợ nhân viên y tế và truyền thông – giáo dục sức khỏe. Tuy nhiên, mức độ triển khai giữa các nhóm dịch vụ có sự khác biệt rõ rệt và tồn tại xu hướng lệch chuẩn về chức năng. Trong đó, nhóm dịch vụ hỗ trợ – tư vấn cho người bệnh và người nhà được triển khai phổ biến và hiệu quả nhất, đóng vai trò trực tiếp trong việc hỗ trợ thủ tục, cung cấp thông tin và ổn định tâm lý ban đầu. Ngược lại, các dịch vụ hỗ trợ nhân viên y tế và truyền thông – giáo dục sức khỏe còn hạn chế, thiếu tính hệ thống và chưa phát huy đầy đủ vai trò trong mô hình CTXH y tế hiện đại. Đặc biệt, các dịch vụ chuyên sâu như tham vấn tâm lý, quản lý ca, hỗ trợ khủng hoảng và điều phối liên ngành hầu như chưa được triển khai đầy đủ, cho thấy dịch vụ CTXH tại Nghệ An hiện vẫn vận hành chủ yếu theo logic hỗ trợ hoạt động của bệnh viện, hơn là theo logic nghề nghiệp của công tác xã hội lâm sàng.

Thứ hai, Đối với người bệnh và người nhà người bệnh, kết quả nghiên cứu chỉ ra rằng họ phải đối mặt với hệ thống khó khăn đa chiều trong quá trình khám chữa bệnh, trong đó khó khăn tài chính là rào cản lớn nhất, tiếp đến là các vấn đề liên quan đến thủ tục hành chính, thiếu thông tin, áp lực tâm lý và gánh nặng chăm sóc. Nhu cầu đối với dịch vụ CTXH được ghi nhận ở mức cao, đặc biệt đối với các dịch vụ hỗ trợ thông tin, tư vấn và hỗ trợ tâm lý. Tuy nhiên, mức độ tiếp cận dịch vụ trên thực tế chưa tương xứng với nhu cầu, nhất là đối với các dịch vụ chuyên sâu và các nhóm người bệnh dễ bị tổn thương như bệnh nặng, điều trị dài ngày hoặc có hoàn cảnh kinh tế khó khăn. Đồng thời, sự khác biệt về khó khăn, nhu cầu và khả năng tiếp cận giữa các nhóm đối tượng cho thấy tồn tại những bất bình đẳng đáng kể trong tiếp cận dịch vụ CTXH trong bối cảnh địa phương.

Thứ ba, Từ góc nhìn của người làm CTXH, việc triển khai dịch vụ chịu sự chi phối của một hệ thống các yếu tố liên kết chặt chẽ, trong đó nổi bật là yếu tố tổ chức và quản trị bệnh viện, đặc biệt là quan điểm và định hướng của lãnh đạo; yếu tố chính sách phát triển đội ngũ, bao gồm đào tạo, phân công, môi trường làm việc và cơ chế phối hợp; yếu tố năng lực và điều kiện hành nghề; và yếu tố nhận thức nghề nghiệp. Đáng chú ý, nhận thức nghề nghiệp được xác định là cơ chế trung gian quan trọng, vận hành như một “bộ lọc thực hành”, trực tiếp định hình việc lựa chọn, ưu tiên và tổ chức các hoạt động CTXH trong thực tiễn. Tổng hợp các yếu tố này cho thấy việc triển khai dịch vụ CTXH không chỉ phụ thuộc vào nguồn lực, mà là kết quả của một chuỗi tác động từ tổ chức – chính sách – năng lực – nhận thức nghề nghiệp.

11. Summary of the new findings of the thesis:

Purpose: This dissertation aims to systematically examine the theoretical foundations of hospital social work services and to investigate the difficulties and support needs of patients and their family members during the process of medical examination and treatment. At the same time, the study evaluates the current implementation status and analyzes factors influencing the delivery of three groups of hospital social work services in accordance with Circular No. 43/2015/TT-BYT of the Ministry of Health. Based on empirical findings, the dissertation proposes recommendations to improve and enhance the quality of hospital social work services within the hospital system of Nghe An Province.

Objectives:   Hospital social work services, including (i) counseling and support services for patients and their family members, (ii) support services for healthcare professionals, and (iii) communication and health education services at hospitals in Nghe An Province, as stipulated under the task framework of Circular No. 43/2015/TT-BYT.

The major results

­  First, the findings indicate that hospital social work services in Nghệ An have been implemented in accordance with the three service clusters stipulated in Circular No. 43/2015/TT-BYT, namely: (i) support and counseling for patients and their families, (ii) support for healthcare professionals, and (iii) health communication and education. However, the level of implementation varies substantially across these clusters, revealing a clear functional imbalance. Among them, patient- and family-centered support and counseling services are the most widely implemented and effective, playing a direct role in facilitating administrative procedures, providing essential information, and stabilizing patients’ initial psychological states. In contrast, services aimed at supporting healthcare staff and those related to health communication and education remain limited, fragmented, and insufficiently systematized, thereby failing to realize their full potential within a modern medical social work framework. Notably, specialized interventions—such as psychological counseling, case management, crisis intervention, and interdisciplinary coordination—are largely underdeveloped. This suggests that social work services in Nghệ An continue to operate predominantly under a hospital-support logic rather than a clinical social work professional logic.

Second, from the perspective of patients and their caregivers, the study reveals that they encounter a multidimensional set of challenges throughout the healthcare process. Financial hardship emerges as the most significant barrier, followed by administrative complexities, information deficits, psychological distress, and caregiving burdens. The demand for social work services is consistently high, particularly for information provision, counseling, and psychosocial support. However, actual service accessibility remains disproportionate to these needs, especially with regard to specialized services and among vulnerable groups, including severely ill patients, those undergoing long-term treatment, and individuals facing economic hardship. Furthermore, variations in difficulties, needs, and access across different population groups indicate the presence of significant inequities in accessing social work services within the local context.

Third, from the perspective of social workers, service implementation is shaped by an interconnected system of determinants. Prominent among these are organizational and hospital governance factors—particularly leadership perspectives and strategic orientations; workforce development policies, including training, task allocation, working conditions, and coordination mechanisms; professional capacity and practice conditions; and professional awareness. Importantly, professional awareness functions as a critical mediating mechanism, operating as a “practice filter” that directly shapes how social work activities are selected, prioritized, and organized in practice. Taken together, these findings suggest that the implementation of social work services is not merely a function of resource availability, but rather the outcome of a dynamic interaction among organizational structures, policy frameworks, professional capacity, and professional cognition.
