11. Tóm tắt các kết quả mới của luận án
- Ứng dụng mô hình Matrix trong hỗ trợ điều trị bệnh nhân methadone có sử dụng ATS, về thực trạng điều trị bệnh nhân methadone tại địa bàn nghiên cứu quận 4 và quận 8 thành phố Hồ Chí Minh, các đặc điểm về nhân khẩu học xã hội của tổng thể bệnh nhân đang điều trị tại 2 phòng khám methadone trước khi tiến hành lựa chọn nhóm bệnh nhân can thiệp và nhóm chứng. Kết quả can thiệp sau 16 tuần cho thấy hiệu quả có sự khác biệt giữa hai nhóm chứng và nhóm can thiệp về tình trạng sử dụng chất, về hành vi xã hội về cảm nhận chất lượng cuộc sống, động lực cai nghiện ma tuý và đặc biệt tăng cường tuân thủ điều trị đều cho kết quả tốt hơn nhóm không được can thiệp. Kết quả này tiếp tục được theo dõi ở mốc thời gian 1 tháng và 3 tháng sau can thiệp cho thấy những bệnh nhân ở nhóm can thiệp tiếp tục duy trì trong tuân thủ điều trị cũng như tình trạng sử dụng các chất gây nghiện đặc biệt là sử dụng ATS đều cho thấy kết quả tốt hơn nhóm chứng. Từ những kết quả trên, tiếp tục thảo luận về việc ứng dụng mô hình Matrix tại địa bàn nghiên cứu và đưa ra những bàn luận về tính khả thi có thể triển khai một cách rộng rãi mô hình này trong thời gian tới.
- Trong quá trình triển khai mô hình can thiệp, Luận án chỉ ra mô hình này đặc biệt phù hợp và khả thi để triển khai rộng rãi ở Việt Nam trong việc hỗ trợ điều trị nghiện ATS nói chung và những bệnh nhân đang điều trị methadone có sử dụng ATS nói riêng. Việc áp dụng Công tác xã hội nhóm trong mô hình can thiệp Matrix là rất thích hợp trong bối cảnh của Việt Nam hiện nay. Từ thực tế đó Luận án đề xuất sự cần thiết phải chuẩn hoá nội dung chương trình Matrix, để mô hình này có thể áp dụng trong thời gian tới tại các Cơ sở cai nghiện ma tuý, các cơ sở điều trị nghiện thay thế bằng thuốc Methadone…cần xem xét việc tuyển dụng nhân viên Công tác xã hội, Tâm lý học, Xã hội học…để đào tạo, nâng cao năng lực cho đội ngũ nhân viên này sử dụng chương trình Matrix trong công việc hỗ trợ điều trị nghiện. Chương trình can thiệp theo mô hình Matrix nên lồng ghép đào tạo trong các Trường Đại học dành cho sinh viên chuyên ngành Công tác xã hội, Tâm lý học, Xã hội học, Y học dự phòng, Y tế công cộng để nâng cao chất lượng nguồn nhân lực điều trị nghiện cho Việt Nam trong thời gian tới.

- Ứng dụng mô hình Matrix trong hỗ trợ điều trị bệnh nhân methadone có sử dụng ATS, về thực trạng điều trị bệnh nhân methadone tại địa bàn nghiên cứu quận 4 và quận 8 thành phố Hồ Chí Minh, các đặc điểm về nhân khẩu học xã hội của tổng thể bệnh nhân đang điều trị tại 2 phòng khám methadone trước khi tiến hành lựa chọn nhóm bệnh nhân can thiệp và nhóm chứng. Kết quả can thiệp sau 16 tuần cho thấy hiệu quả có sự khác biệt giữa hai nhóm chứng và nhóm can thiệp về tình trạng sử dụng chất, về hành vi xã hội về cảm nhận chất lượng cuộc sống, động lực cai nghiện ma tuý và đặc biệt tăng cường tuân thủ điều trị đều cho kết quả tốt hơn nhóm không được can thiệp. Kết quả này tiếp tục được theo dõi ở mốc thời gian 1 tháng và 3 tháng sau can thiệp cho thấy những bệnh nhân ở nhóm can thiệp tiếp tục duy trì trong tuân thủ điều trị cũng như tình trạng sử dụng các chất gây nghiện đặc biệt là sử dụng ATS đều cho thấy kết quả tốt hơn nhóm chứng. Từ những kết quả trên, tiếp tục thảo luận về việc ứng dụng mô hình Matrix tại địa bàn nghiên cứu và đưa ra những bàn luận về tính khả thi có thể triển khai một cách rộng rãi mô hình này trong thời gian tới.

Từ thực tế đó Luận án đề xuất sự cần thiết phải chuẩn hoá nội dung chương trình Matrix, để mô hình này có thể áp dụng trong thời gian tới tại các Cơ sở cai nghiện ma tuý, các cơ sở điều trị nghiện thay thế bằng thuốc Methadone…cần xem xét việc tuyển dụng nhân viên Công tác xã hội, Tâm lý học, Xã hội học…để đào tạo, nâng cao năng lực cho đội ngũ nhân viên này sử dụng chương trình Matrix trong công việc hỗ trợ điều trị nghiện. 11. Summary of the new findings of the thesis:
- The thesis examines the application of the Matrix model in enhancing the treatment of methadone maintenance treatment (MMT) patients using ATS, and provides information on the current treatment status and demographic characteristics of MMT patients in the study area including MMT centers of District 4 and District 8 in Ho Chi Minh City before the intervention and control group selection phase. The results after 16 weeks showed that there were differences between the two groups: the intervention group was found to have better results in terms of substance use status, social behavior, perceived life quality, motivation for quitting drug, and adherence to treatment than the control group. These results were continued to be monitored at the periods of 1 month and 3 months after the intervention, showing that the patients in the intervention group continued to maintain their adherence to treatment and their status of using special substances, especially ATS better than those in the control group. Besides discussing the above results, the thesis continues to examine the application of the Matrix model in the study area and its feasibility.
- Based on the process of implementing the intervention model, the thesis showed that this model was particularly suitable and feasible to be widely implemented in Vietnam in enhancing the treatment of ATS addiction in general and in supporting MMT patients using ATS in particular. The application of social work with groups in the Matrix intervention model is very suitable in the current context of Vietnam. Established on that fact, the thesis proposes the need to standardize the materials of the Matrix model, so that this model can be applied widely in the near future at drug addiction treatment facilities and MMT centers, the need to recruit more Social worker, Psychologist and Sociologist in the field, and the need to train and improve the competence of personnel so that they can apply the Matrix model in their work. Moreover, the thesis proproses that the Matrix intervention model should be integrated in the study curriculum of students majoring in Social Work, Psychology, Sociology, Preventive Medicine, and Public Health to elevate the quality of human resources in addiction treatment in Vietnam in the future.
